Campus Transfer Request Form
Use this form to request a transfer to another WCU campus.

Challenge Exam Request Form
Use this form to request for an opportunity to challenge a course. Please review the Challenge Exam policy and consult with your advisor.

Change of Course Enrollment Status Request
*For Distance Education program only – Use this form to request for a change in enrollment status, ex: Full Time to Part Time

Course Audit Request Form
Use this form to request to audit a course

Course Section Change Request Form
Use this form to request for a change in course section

Course Sequence Change Request
Use this form to request for a change in course schedule that is not in alignment with a predetermined course sequence

Course/University Withdrawal Form
Use this form to withdraw from the course or the University

Duplicate Diploma Request
Use this form to request for a duplicate diploma

Enrollment Verification Request Form
Use this form to request a verification of your enrollment at ¹ú²úÖ÷²¥. Please note that for faster turnaround, you may download a copy of your Enrollment Verification letter through your student portal.

Exam Date Change Form
Use this form to request a change in exam date (prelicensure nursing programs only)

Final Course Grade Appeal – Step 1
Use this form to request a grade appeal with your faculty

Final Course Grade Appeal – Step 2
Use this form to request a grade appeal with the program dean after a denial from your faculty

Intercampus Registration Form
Use this form to request register course(s) at another campus

Leave of Absence Request Form
Use this form to request a Leave of Absence.

Letter of Recommendation Form
Use this form to request a Letter of Recommendation

Overload Request Form
Use this form to request a course overload

Name Change Form
Use this form to request an official change to your legal name. Email or address changes can be completed on the student portal.

Petition for Exception to Academic Policy
Use this form to request for an exception to a policy outlined in the University catalog

Program Transfer
Use this form to request for a program transfer within the same campus or register for a course at another course.

Reentry/LOA Return Request Form (Ground programs)
Use this form to request for a reentry into the University after withdrawal or getting clearance to return from LOA. Consultation with your advisor is required.

Request for Incomplete Grade
Use this form to request an Incomplete Grade

Student Request Form
Use this form to request for a copy of your attendance history for specific courses, a copy of specific documents from your record/student file

Transcript Request Form
Use this form ONLY if you attended ¹ú²úÖ÷²¥ prior to 1997. Otherwise, please place a transcript order on National Student Clearinghouse
